General information
- Brazilian citizens who died in the State of Israel may have their death registered by means of a declaration by a family member or authorized funeral agent, who must come in person to the Consular Section.
- If a funeral agent is representing the family, an authorization form must be presented, filled out by a family member, with a notarized signature recognized by a notary office in Brazil (cartório) or by a Consular Section. The form must be accompanied by a simple copy of the family member's identification document.
- The consular death certificate must later be transcribed, in Brazil, to the 1st Civil Registry Office in order for it to have effect in Brazil or any other Consular Section abroad.
- The issuance of the first copy of the certificate is free of charge. The issuance of a second copy costs 20.00 NIS per copy.
- The consular death certificate request must be made through the e-consular system. After validation by the system, an appointment must be scheduled for the declarant to appear at the Consular Section.

The form must be completed by the declarant. It can be filled out by hand or on the computer, leaving the date and signature fields blank (which will be filled in at the time of service). It is essential that, at the time of service, the original form, duly completed, is presented. If a funeral agent is representing the family, an authorization form must be presented, filled out by a family member, with a notarized signature recognized by a notary office in Brazil (cartório) or by a Consular Section. Israeli death certificate |
The Israeli death certificate (Tehuda Ptirá), issued by the Ministry of the Interior (Misrad HaPnim) must be presented. The Consular Sector can only accept documents in Portuguese, Spanish or English. Bilingual documents (English/Hebrew or English/Arabic) may be accepted, provided that the essential data are in English and that there is no discrepancy in the spelling of names. If the document is written only in Hebrew, a notarized translation of the death certificate must be presented in English, Spanish or Portuguese. |

Medical report with the cause of death |
This document must be presented only when this information is not included in the foreign death certificate. It should be noted that the Ministry of the Interior only adds the cause of death upon request. If the medical report is in Hebrew, however the cause of death is declared in English, the document can be accepted without translation. If the document is written only in Hebrew, a notarized translation of the medical report in English, Spanish or Portuguese must be presented. |
