Consular birth registration based on a foreign birth certificate – between 16 and 18 years of age
For the registration of minors between 16 and 18 years of age, the presence of the registrant at the Consular Office is required. The registrant will act as the declarant of their own birth and must be accompanied by one of their legal guardians. The assisting legal guardian and two witnesses must complete and sign the application form and the consular birth registration record.

*Any document that does not allow full identification of the holder, whether due to age, erasure, or damage, will not be accepted and the request will be denied.

Completed and signed birth registration application form |
The form (click here) must be completed on the computer before being printed and signed by the declarant (mother, father, or legal guardian) of Brazilian nationality. It is essential that, at the time of the appointment, the form is already duly completed and signed.

In case of change of the parents’ names after the child’s birth registration, provide a document (Brazilian and/or foreign) that proves the change (e.g., marriage certificate).
