Consular birth registration based on a foreign birth certificate – between 16 and 18 years of age

For the registration of minors between 16 and 18 years of age, the presence of the registrant at the Consular Office is required. The registrant will act as the declarant of their own birth and must be accompanied by one of their legal guardians. The assisting legal guardian and two witnesses must complete and sign the application form and the consular birth registration record.

Required documentation

*Any document that does not allow full identification of the holder, whether due to age, erasure, or damage, will not be accepted and the request will be denied.

Documentation		Notes
1	Completed and signed birth registration application form	The form (click here) must be completed on the computer before being printed and signed by the declarant (mother, father, or legal guardian) of Brazilian nationality. It is essential that, at the time of the appointment, the form is already duly completed and signed.
2	Original birth documents issued by the foreign authority	Subject to possible apostille or consular legalization requirements.
3	Documents proving the Brazilian nationality of the Brazilian parent	Original and copy of one of the following: Brazilian birth certificate; Brazilian marriage certificate, provided it explicitly mentions nationality; or Certificate of naturalization;
4	Brazilian identification documents of the Brazilian parent	Original and copy of one of the following: Brazilian passport, even if expired, issued by the Federal Police or the Ministry of Foreign Affairs; Identity card (RG) issued by a State Public Security Secretariat; Professional ID card issued by a public agency or professional regulatory body recognized by federal law as a valid identity document throughout Brazil; National Driver’s License ― CNH, even if expired, issued by DETRAN; Indigenous identity card; FUNAI declaration attesting to the authenticity of the personal data of a non-integrated indigenous person; Work and Social Security Card ― CTPS; or Digital identification document recognized by federal law as valid throughout Brazil, such as CNH or DNI.
5	Documents proving the nationality and identity of the other parent	Original and copy of the following: If Brazilian: the same documents listed in items 3 and 4; or If foreign: valid passport or local ID document issued by the competent authority, and a document proving parentage.
6	Document proving name change	In case of change of the parents’ names after the child’s birth registration, provide a document (Brazilian and/or foreign) that proves the change (e.g., marriage certificate).
