VITEM II - Health Treatment
Health Treatment visas are issued to foreign patients and their accompanying parties, who travel to Brazil for the purpose of receiving medical treatment.
⚠️ Prior authorization from The Ministry of Foreign Affairs’ Headquarters (which is internally processed by SERE/DIM) is required.
⚠️ The exercise of remunerated activities in Brazil is not allowed.
⚠️ For stays of up to ninety (90) days, you may apply for a Visitor Visa (VIVIS).
General requirements

| 4 | Police clearance issued within the last 3 months; ⚠️ Only for those who are 18 years old or older. |
| 5 | Proof of financial capacity to cover general expenses during the intended stay in Brazil (e.g., copies of local bank statement covering the last three months); |

Proof that you (or your sponsor) have sufficient funds to pay for your medical expenses in Brazil, by means of:

| 7 | * Medical prescription or report for the procedure/treatment; |
| 8 | * Estimate of the treatment costs, including the ones associated with hospitalization, doctor’s fees, and other medical expenses; |
| 9 | * Written statement from the physician in charge or from the hospital/clinic, confirming that the treatment will not economically impact the Brazilian Unified Health System (SUS), except in case of reimbursement; |
| 10 | ** Copy of birth certificate, coupled with a translation thereof into English, signed by the translator (applicant may not be the translator); |
| 11 | ** Copy of the Japan residence card (Zairyu Card); ⚠️ For non-Japanese nationals only. |
| 12 | Visa fee. |
REMARKS:
* These documents must be signed and the signature must be recognized by a Notary Public in Brazil (firma reconhecida em cartório).
** The original document must be submitted together with a copy. The original document shall be returned to the applicant.
Required documents for the accompanying parties

REMARKS:

For people under 18 years old
All of the above-captioned documents, as applicable, as well as the following ones:

Signature of both parents (both father and mother or guardian) is required in the signature field of the application form. |
