Birth registration based on the Japanese certificate

What it is

Birth certificate issued by the Consulate based on the Japanese birth certificate.

How to apply

With the necessary documents in hand, schedule the service through the e-Consular system. Check out the system tutorial here. Once the appointment has been made, you will need to go to the Consulate in person and present the original documents.

Documents required

1	Form	Birth registration form completed and signed by parents ⚠️ Disable your browser's pop-up blocker to open the form.
2	Documents issued by the Japanese city hall	Shusshō Juri Shōmeisho (出生受理証明書) Shusshō Todoke Kisai Jikō Shōmeisho (出生届記載事項証明書)
3	Parents' identity card	Brazilian parents: Original and copy of passport valid or expired less than 2 years ago, or ID card, or Brazilian driver's license ⚠️ Brazilian parents with dual nationality must also present Koseki Tohon (戸籍謄本) issued less than three months ago and Japanese passport (if any) Japanese parents: Original and copy of valid passport, or driver's license, or My Number card (個人番号カード); Koseki Tōhon (戸籍謄本) issued less than three months ago Parents of other nationalities: Original and copy of valid passport
4	Parents' birth and/or marriage certificate (as appropriate)	If married: a copy of the marriage certificate ⚠️ The Brazilian marriage registration of the parents, if applicable, should preferably precede the birth registration of the children. If unmarried: a copy of the birth certificate If divorced: a copy of the marriage certificate with divorce annotation ⚠️ Certificates submitted must be in English, Spanish or Romanized.

Additional documents (for those over 12 years old)

5	Identity document of the person to be registered	School card, or foreign passport, or My Number card (個人番号カード)
6	Witnesses' identity documents	Original and copy of identity documents
7	Data form for the two witnesses	Completed and signed by the witnesses ⚠️ Witnesses can be Brazilian or foreign and relatives of any degree of the person to be registered.
