VISA REQUIREMENTS CONSULAR INFORMATION SHEET
A form duly completed and signed by the applicant on the official website:

Then, proceed to book your appointment for submission at the following link:
https://ec-lagos.itamaraty.gov.br/
Kindly follow the instructions as stated on the attached E-Consular.
- A manual Visa Application Form must be filled out;
- An international passport valid for at least six months 2 recent colored passport photographs on a white background;
- Birth Certificate or Sworn Affidavit of Birth;
- If married, a copy of the Marriage Certificate;
- Copy of passport Bio-Data page;
- Introduction letter from Employer/Organization/ School;
- Self-Introduction Letter;
- Invitation letter stamped at the Cartório (Notary) in Brazil by "Reconhenço por autenticidade" with a copy of the signatory’s ID or the digital signature from https://assinador.iti.br/assinatura/index.xhtml, where the document should be uploaded as a PDF, for validation in the program;
- If self-employed, Certificate of Incorporation of the company and complete Memorandum &Articles of Association;
- Original personal/salary bank statement for the last three months;
- Valid hotel reservation;
- Flight reservation;
- Itinerary and breakdown of the intended trip to Brazil Print out your appointment confirmation instructions to send your application via courier, which MUST include a return courier bag.
- If being sponsored, the Sponsor's bank statement for the last three months & a sponsorship letter;
In the case of sponsorship by a private person:
- Notarized letter of sponsorship informing the relation of the sponsor to the applicant and the specific expenses to be covered by the sponsorship (a figure in USD must be informed for travel expenses of the applicant) – this letter must be notarized.
- Valid ID (Passport or other).
- Birth certificate (or the equivalent).
- Marriage certificate (or the equivalent) - if the case.
- Proof of address.
- Proof of business (CAC), labor occupation, job, retirement and/or proof of income.
- Personal bank account statements (up to 3 pages: the first, the last, and a random page) – the complete set must be delivered with the passport).
In the case of sponsorship by the employer (work/business related travel only):
- Letter from the employer, informing the details of business meeting or assignments in Brazil, plus notarized copy of the employer´s ID.
- Company´s CAC and proof of address.
- Bank´s letter of recommendation and account statements (3 pages).
- Brazilian business partner invitation letter (informing CNPJ, contact data and having certified signature), plus proof of address and authenticated copy of inviting person´s Brazilian ID.
ADDITIONAL INFORMATION: FOR MINORS UNDER 18 YEARS OLD
- Two Parental Authorization Forms duly filled out and notarized either by the Cartório (Notary) in Brazil or by any of the approved Notaries in Nigeria;
- Parental consent letter stamped at the Federal Ministry of Foreign Affairs;
- Birth Certificate.
ADDITIONAL: FOR PERSONS GOING TO BUSINESS
- Certificate of Incorporation of the company and complete Memorandum &Articles of Association;
- Proof of Tax returns; Invitation letter in Portuguese from the host company or organization, which must include the CNPJ (Company registration details);
- Proof of previous transactions in Brazil (if applicable) Company Bank Statement for the last three months.
IMPORTANT INFORMATION
1. Applicants MUST upload their Passport Photograph, Signature, International Passport Bio-Data Page, and ALL other documents as possible when filling out the online form.
2. All data (names, date of birth, etc.) must be exactly like stated on all documents (passport, birth/marriage certificate, etc.)
3. Passport photographs must be GLUED on the forms before arriving at the Consulate.
4. All applications must be submitted with the International Passport (old/previous included); applications without the International Passport will not be collected.

6. All visa processing fees are non-refundable and must be paid before arriving at the Consulate.
