Passo 1: Acesse a plataforma Saúde Digital MT https://telessaudemt.com.br
Passo 2: Informe seu login e senha para Acessar o sistema.
Caso não possua acesso, entre em contato com o suporte da Saúde Digital MT aqui para solicitar seu cadastro.
Passo 3: Selecione a opção Cadastro
Passo 4:Clique em Prontuário
Passo 5: Informe o CPF do paciente SUS
Passo 6: Clique em Buscar e visualize o histórico de Atendimentos do paciente SUS
