Passo 1 - Fazer a leitura sobre as regras e valores do plano, disponíveis no site do Mato Grosso Saúde Acessar site aqui.
Passo 2 - Verificar quais os documentos necessários para aderir ao plano.
Passo 3 - Fazer download do formulário de adesão e preencher.
Passo 4 - Enviar formulário preenchido e a cópia dos documentos solicitados no e-mail: atendimento@mtsaude.mt.gov.br.
