Passo 2 - Verificar quais os documentos necessários para solicitar a exclusão do plano.
Passo 3 - Fazer download do formulário de exclusão e preencher.
Passo 4 - Justificar motivo de saída do plano.
Passo 5 - Enviar o formulário preenchido e a cópia dos documentos solicitados no e-mail: atendimento@mtsaude.mt.gov.br.
