Passo 1 - Comparecer a sede da Procuradoria Geral de Justiça do Estado de Mato Grosso - PGJ.
Passo 2 - Expor os fatos e assinar termo de declaração.

Passo 1 - Acessar o site pelo link: https://www.mpmt.mp.br
Passo 2 - Preencher o formulário.
Passo 3 - Anexar documento se houver (documentos de até 50 mb nos formatos PDF, Word, Excel, JPG, GIF, Vídeo e Áudio. Para anexar é só clicar no botão “anexar arquivos” no final do formulário).
Passo 4 - Enviar mensagem (por meio do campo complementar).
