Steps for carrying out this service

Atendimento Presencial
Passo 1 - Preencher o formulário anexo em nome da pessoa que consta do documento de arrecadação (boleto do DETRAN/MT) ou no nome que consta no comprovante de pagamento e imprimir em 02 (duas) vias.
Passo 2 - Anexar cópia dos documentos.
Passo 3 - Protocolar nas Unidades do DETRAN/MT onde houver protocolo. Para as localidades onde não existe unidade do DETRAN/MT, encaminhar via Correios.
