Steps for carrying out this service

Atendimento Semipresencial
Para Aférese terapêutica
Passo 1 - Solicitar o procedimento no ambulatório pelo email gmt@ses.mt.gov.br
Passo 2 - Aguardar parecer médico do MT- Hemocentro.
Passo 3 - Receber as orientações necessárias após parecer médico favorável.
Passo 4 - Organizar junto ao ambulatório a realização do procedimento.
Para Sangria terapêutica
Passo 1 - Solicitar o procedimento na Central de Regulação/ SISREG. (teleconsulta médica na atenção especializada/ Hemato adulto).
Passo 2 - Aguardar liberação da vaga pelo SISREG.
Passo 3 - Encaminhar no email gmt@ses.mt.gov.br a autorização da Central de Regulação/ SISREG, resultados de exames, pedido médico de sangria, documento oficial com foto, CNS e CPF do paciente.
Passo 4 - Aguardar retorno do ambulatório com as orientações médicas necessárias e agendamento do procedimento.
