Health Treatment (VITEM II)
a) General rules
- It is intended for foreign nationals travelling to Brazil to undertake health treatments expected to last more than 90 days.
- For stays with duration of up to 90 days, the applicant must request a Visitor Visa (VIVIS). Bahamian passport holders are exempt from Visitor Visas (VIVIS); therefore, they do not need to obtain a visa for this specific purpouse. Other nationalities must check the visa requirements table.
b) Required documents
Please note: the original documents listed below must be presented during the interview.

Must be filled in online (click here). Print and bring the Visa Request Form Receipt (RER) to the appointment. It is very important to fill in the parents' full names, as displayed in the applicant's birth certificate. Please note: the Embassy will only accept applications in which the required documents listed on this table have been uploaded to the form. |

Birth certificate - Full form* |
In the absence of the full birth certificate, the applicant may submit another official document that contains the parents’ full names. *Attention: submitting this document is intended to prevent any discrepancies between the information contained in the visa application and the birth certificate itself. As the birth certificate will have to be presented during the mandatory registration of the visa with the Federal Police in Brazil, any inconsistent data will preclude the conclusion of this process. Thus, the presentation of the birth certificate during the visa application stage abroad is not compulsory and will not prevent the visa from being issued, but is highly recommended in order to avoid possible inconveniences. |

Police check (applicants aged over 18 years old) |
In the Bahamas, police records are issued by the Royal Bahamas Police Force. Police checks are valid for up to 3 months from the date of issue. Please note: if the applicant has lived in multiple countries within the last 12 months, a police check from each country of residence must be provided. They must also be duly legalized (contain an apostille) by the competent authority in the issuing country and, if necessary, translated by a certified translator. |

Air tickets |
The applicant must submit either a booking confirmation (paid ticket or e-ticket) or a booking reservation (or letter from a travel agent on the applicant's behalf), containing the complete itinerary, flight numbers and arrival / departure dates. If you do not have flight reservation, a return declaration form must be filled out. |

Proof of sufficient financial means, compatible with the entire stay in Brazil |

Original treatment medical referral letter or medical report |
A letter must printed on official letterhead of the health professional or healthcare unit and contain at least the following information: a) The applicant's full personal details and the healthcare unit or medical professional's details; b) Description of the patient's condition and proposed treatment plan; c) Estimated duration of treatment; d) medical quote from the doctor or institution in Brazil, with signature(s) notarized in Brazil. |

Travel authorization (only for applicants under 18 years old) |
Authorization signed by the parents / legal guardians in case of child or adolescent, as well as the details of the person who will be responsible for the minor in Brazil. |

Statement of no public funds involvement |
Statement issued by the doctor, hospital, or clinic responsible for the treatment, affirming that the procedure will not impose any cost on the Brazilian Unified Health System (Sistema Único de Saúde – SUS). The statement must bear signatures notarized in Brazil. |
