Required documents and instructions:

• Visa request form, signed and with passport size (2” x 2”) picture, with white background, attached
• Valid Passport
• Birth Certificate + Apostille (or legalization)
• Proof of legal residence (if applicable)
• Recent FBI Background check, NOT OLDER THAN 03 MONTHS
• Proof of residence
• Health insurance with coverage in Brazil, during your stay
• Airline ticket or reservation 
• Proof of retirement and the ability to transfer to Brazil, in U$, the MINIMUM amount of U$2,000.00 or more, including other sources of income
• Cost: $290.00 - payable in USPS Money Order or US Debit Card only (FEES ARE NOT REFUNDABLE)
