Lost, stolen or damaged passport
PASSPORT TO REPLACE LOST, STOLEN OR DAMAGED TRAVEL DOCUMENT
The Consulate of Brazil in Chicago issues passports in place of a lost, stolen, or damaged passport, upon presenting all original supporting documents. Copies are not accepted. All passport applications must undergo previous document screening, performed online through the E-Consular Platform.
FORMS
To apply, you must complete at least THREE different forms:
- formulário RER (Application Form Receipt);
- formulário de comunicação de extravio, furto ou passaporte danificado (Lost, stolen or damaged passport form) and;
- formulário online da plataforma E-Consular (E-Consular Form).
If the passport to be replaced is that of a minor, an extra form must also be submitted (uploaded):
- formulário de autorização para concessão de passaporte para menor (Consent Form for Minors with travel authorization).
REQUEST THROUGH E-CONSULAR
The Consulate will only analyze passport applications after all forms are properly completed and the corroborating documents are uploaded through the e-Consular platform.
REQUIRED DOCUMENTS
IMPORTANT: All documents issued at Brazilian Consulates abroad now require the presentation of the CPF number for the document holder (the physical CPF card, in plastic format, is no longer issued; the number is sufficient to make the request). Please have your CPF number with you when submitting a request through E-Consular.

For in-person applications, applicants must bring, along with the required documentation, ONE empty return envelope, Priority or Priority Express, from the USPS (United States Postal Service), prepaid and self-addressed, with a tracking number, for the Consulate to use to mail the new passport. For mail-in applications, applicants must send TWO empty return envelopes, Priority or Priority Express, from the USPS (United States Postal Service), prepaid and self-addressed, with a tracking number. One envelope will be used to return the original documents, and the other envelope will be used to mail the new passport after it is printed in Brazil and received by the Consulate. ATTENTION: Do not use any mail carrier other than USPS. The prepaid "label" with a tracking number can be purchased at a USPS location. ATTENTION: We do not accept labels purchased through the Click-N-Ship option for passport applications, as they expire before the service can be completed. |
When the damage does not impair the identification of the passport holder, please follow the guidelines for passports for individuals 18 and older or for minors under 18, where applicable.
When the damage makes it impossible to identify the passport holder, follow the instructions for lost or stolen passports.
PROCESSING TIMES
